The Role

We are seeking a compassionate and experienced Coordinator to lead the delivery of our innovative Pause Program, supporting people living with dementia and their carers through tailored respite experiences. This role focuses on enhancing wellbeing by coordinating person-centred care plans and facilitating meaningful respite at various stages of the dementia journey. You’ll work closely with clients and families to create flexible support options that promote living well at home for longer.

This position is offered as a fixed-term, part time role, working 46 hours per fortnight, ending on the 30th of July 2027. Role is based in our Subiaco Head office working days are as follow Monday, Wednesday and Friday 8:30 to 4:30.

 

Key responsibilities:

Program Development and Implementation

  • Use clinical skills to support the Manager, Pause Program, in implementing the program with clients and carers.
  • Engage with participants (carers and people living with dementia) to ensure timely and appropriate respite support.
  • Conduct clinical assessments to identify tailored informal and formal respite options.
  • Use validated tools to support reflection on self-care and wellbeing.
  • Apply pilot outcomes to shape a person-centred dementia respite model.
  • Provide individual and group education and support on all aspects of respite.
  • Co-design and implement respite planning tools and procedures with participants.
  • Develop personalised Respite Plans in collaboration with carers and clients.
  • Build community capacity through partnerships with internal and external stakeholders.
  • Maintain professional knowledge to ensure high-quality respite support.

Communication and Teamwork

  • Work collaboratively with clients, carers, and stakeholders to ensure an integrated respite model.
  • Participate in program meetings, briefings, and stakeholder updates.

Operations, Administration and Documentation

  • Record participant engagement and relevant information
  • Maintain accurate records using electronic systems and report outputs in line with funding requirements.
  • Liaise with the Operations Support Officer to manage administrative functions.
  • Support the development and implementation of the Dementia Respite Support Guide.
  • Complete administrative tasks related to grant requirements and the role.

 

About you

You are a qualified health professional with strong clinical assessment skills and a passion for supporting people living with dementia and their carers. You thrive in collaborative environments, have experience in delivering person-centred care, and can translate program learnings into practical, innovative respite solutions. Your excellent communication, planning, and documentation skills enable you to work effectively with clients, families, and stakeholders to improve wellbeing and support living well at home.

 

Essential Criteria:

  • A formal qualification in a related field, e.g. allied health, nursing.
  • Experience supporting people living with dementia.
  • Evidence of Current Flu vaccinations per Amana Living policies
  • Ability to obtain a National Police Clearance (within 6-months validity)
  • Successful completion of pre-employment physical and reference checks
  • Current APHRA Registration without restrictions, or equivalent
  • Western Australian C Class Driver’s License.
