The Role:

The Lifestyle Coordinator is responsible for ensuring the identified lifestyle needs and choices of our residents are captured and reflected in care plans and appropriate activity programming that promotes engagement, social interaction and individual wellbeing. This is a permanent part time role offering 50 hours per fortnight.  Monday to Friday 0900 - 1400 

 

Key Responsibilities:

  • Consults and liaises with the resident, appropriate family members/friends and the multidisciplinary team to identify residents’ leisure & lifestyle preferences, needs and interests.
  • Gathers information regarding residents’ lifestyle needs, social history and individual preferences and utilises this to create goals of care in consultation with the resident and their chosen representatives.
  • Plans, develops and implements a comprehensive and balanced activity program that caters to a diverse range of interests and abilities, promoting social, physical, cognitive, and emotional wellbeing.
  • Coordinates and oversees the daily tasks of therapy assistants and volunteers to ensure seamless program execution and resident engagement.
  • Establishes and maintains connections with local community organisations, schools, and volunteers to enrich the residents' experiences and promote community involvement.
  • Organises outings, visits, and events that connect residents with the broader community.
  • Ensures that the activity program aligns with best practice, evidence-based approaches, and regulatory requirements.
  • Completes regular evaluation of the effectiveness of the activities to ensure the activity program continue to meet each resident’s needs.
  • Completes regular evaluation of resident participation and engagement to ensure their leisure and lifestyle needs are being met.
  • Contributes to regular quality improvement projects related to the activities program.
  • Monitors the use of the Leisure and Lifestyle resources and budget in accordance with program guidelines and in collaboration with the Service Manager.

 

Essential Criteria:

  • Minimum of Certificate IV in Allied Health or related qualification with an understanding of aged care
  • Experience developing innovative, enjoyable and resident-centred lifestyle programs, and coordinating activities in a residential environment
  • Proven ability to document and evaluate activity and/or lifestyle programs
  • Knowledge of the resident classification scale, accreditation and continuous improvement
  • Previous experience coordinating and mentoring therapy assistant staff
  • Well-developed computer skills
  • Valid Police Clearance less than 6 months old (or ability to obtain at interview stage)
  • Successful completion of pre medical form and reference checks
  • Evidence of Flu (2025) and COVID Vaccinations (including Booster if eligible)
  • Willingness to attend a paid 5 day induction (Monday – Friday) at our Rivervale Training Institute.
