The Role

The Pause Program is a pilot initiative aimed at enhancing the wellbeing of both family carers and people living with dementia by offering tailored respite experiences. The coordinator supports this by developing person-centred care plans and working closely with clients and carers to deliver flexible respite options. The goal is to improve outcomes and help people with dementia remain living well at home for longer.

 

This position is offered as a 7-month fixed-term, part-time role, working 75 hours per fortnight. Hours are 8:30am to 4:30pm, based at our Head Office in Subiaco, with flexibility on which weekdays (Monday to Friday) are worked for the right candidate.

 

Key responsibilities:

Program Development and Implementation

  • Support the Manager in implementing the Pause Program with clients and carers.
  • Engage with participants to provide timely, person-centred dementia respite support.
  • Identify and coordinate informal and formal respite options tailored to individual needs.
  • Use validated tools to support self-care and wellbeing reflection.
  • Contribute to the development of a respite model based on pilot outcomes.
  • Deliver individual and group education on respite and dementia care.
  • Co-design and implement respite planning tools with carers and clients.
  • Create personalised Respite Plans in collaboration with carers and clients.
  • Build partnerships with internal/external stakeholders to strengthen community capacity.
  • Maintain professional knowledge to ensure quality support.

Communication and Teamwork

  • Work with clients, carers, and stakeholders to ensure coordinated care.
  • Contribute to team meetings and provide program updates as required.

Operations, Administration and Documentation

  • Maintain accurate documentation in line with procedures and standards.
  • Use electronic systems to manage data and report as per funding requirements.
  • Coordinate with the Operations Support Officer for administrative tasks.
  • Support the development and rollout of the Dementia Respite Support Guide.
  • Complete other administrative tasks as required for the role.

 

About you

You have experience working with people living with dementia and their carers, along with a strong understanding of the challenges they face. You’re a confident communicator with well-developed interpersonal, record-keeping, and computer skills. With a background in community education and operational roles, you’re both collaborative and capable of working independently. You bring a professional, proactive attitude, a growth mindset, and a deep sense of empathy, enabling you to build rapport and contribute meaningfully to the wellbeing of others.

 

Essential Criteria:

  • Evidence of COVID-19 and Current Flu vaccinations per Amana Living policies
  • Ability to obtain a National Police Clearance (within 6-months validity)
  • Successful completion of pre-employment form and reference checks
  • A formal qualification in a related field, e.g. allied health, nursing.
  • Current APHRA Registration without restrictions, or equivalent
  • Western Australian C Class Driver’s License.
  • Experience supporting people living with dementia
