About the Role

As a Home Care Advisor, you will play a pivotal role in:

  • Conducting timely and effective assessments that form the foundation of personalised care plans.
  • Promoting a consumer-directed care approach to empower clients to achieve their goals.
  • Linking clients to services and support networks that enhance their independence and well-being.
  • Maintaining compliance with organisational policies, industry standards, and legislative requirements.

This role requires a proactive, client-focused individual who can balance the needs of clients, carers, and service providers while upholding the highest standards of care.

Key Responsibilities

Client Service:

  • Informing clients about the services provided by Amana Living, focusing on the capacity of clients to maintain independence and maximise their level of function.
  • Manage and undertake face-to-face or phone assessment of potential clients
  • Provide information about access, eligibility, referral processes and service options for clients 
  • Informing clients about the support at Home funding structure and Amana Living pricing
  • Undertaking comprehensive client assessments using relevant and current tools and resources available 

Clinical Care:

  • Providing clinical assessment, ensuring high quality care and services are provided to clients

Relationship Management:

  • Develop and maintain positive internal and external relationships that align with organisational objectives and relevant legislation
  • Establishing and maintaining excellent relationships and networking with all service referral bodies, and broader Home Care Sales team

Administration:

  • Actively participating in the development of assessment and care planning documentation/resources and contributing to the continuous improvement of work practices 
  • Ensure appropriate, accurate and confidential documentation and statistical records are maintained to accurately meet statutory and organisatoinal requirements and enable a streamlined, equitable and efficient process. 
  • Regular and accurate reporting of results (Weekly KPIs)

Skills and Abilities:

  • Highly developed interpersonal and communication skills, including the ability to build trust and rapport with diverse clients.
  • Strong problem-solving skills to address client needs and goals effectively.
  • Exceptional customer service experience with a focus on client satisfaction and feedback.
  • Strong organisational and time-management skills with the ability to handle multiple priorities.
  • Proficiency in using Microsoft Office applications and other relevant software systems.

 

Essential Requirements

  • Current National Police Clearance Certificate (no older than 6 months).
  • Proof of current influenza vaccination, as per organisational policies.
  • A valid Western Australian C-Class Driver’s Licence.
  • Successful completion of pre-employment medical and reference checks.
